G3002 covers a monthly, pain-focused package of care for a patient with chronic pain. A physician or other qualified health professional may assess and monitor the pain, use a validated pain rating tool, develop or adjust a patient-centered care plan, manage treatment and medications, and coordinate related behavioral health or other care. Examples include ongoing management of persistent low back pain, neuropathic pain, or chronic pain associated with osteoarthritis. The work may take place in an office or other setting where the practitioner provides the service.

Report G3002 when the practitioner personally spends at least 30 minutes on qualifying chronic pain management and treatment during the calendar month. Documentation should support the time and describe the pain-related assessment, care planning, treatment management, or coordination performed. The monthly package includes activities such as pain assessment, medication management, pain and health literacy, and care coordination; do not count the same work again as a separate service. G3003 is used for qualifying additional time beyond the initial 30 minutes.

When should G3002 be selected instead of G3003?

G3002 represents the initial 30 minutes of qualifying chronic pain management during the calendar month. G3003 is for each additional 15 minutes and is reported with G3002.

What documentation supports G3002?

Record the time personally spent by the physician or qualified health professional during the month and the chronic pain work performed, such as assessment, care-plan adjustment, medication management, or care coordination.

Can clinical staff time count toward G3002?

The 30-minute requirement is based on time personally spent by the physician or other qualified health professional. Do not use clinical staff time to meet that requirement.

Are pain assessment and care coordination separately reported?

Pain rating, treatment planning, medication management, and related care coordination are part of the G3002 monthly package. A separate service requires distinct work beyond the activities included in that package.

Can an office E/M service be reported in the same encounter?

A separately identifiable E/M service may be reported when it is performed and documented apart from the monthly chronic pain management work. Do not count the same work or time toward both services.
