This add-on captures additional wound surface debrided to subcutaneous fat beyond the first 20 square centimeters. Typical wounds include diabetic foot ulcers, pressure injuries, venous stasis ulcers, and dehisced surgical incisions. Debridement may be performed with a scalpel, curette, or scissors. Podiatrists, general and plastic surgeons, and wound care physicians and practitioners perform it in offices, hospital outpatient wound centers, and inpatient settings.

Add the areas actually debrided to subcutaneous depth across wounds, and report 11042 for the first 20 sq cm or less. Report one unit of 11045 for each additional 20 sq cm or part thereof. For example, 45 sq cm debrided to subcutaneous depth equals 11042 plus two units of 11045. Total areas debrided to deeper levels separately under their own codes. Documentation should identify each wound's location, area debrided, deepest tissue removed, instruments used, and type of tissue excised. Which primary code must accompany 11045?

Report 11045 only with 11042, the first 20 sq cm or less of subcutaneous debridement. Muscle/fascia and bone debridement have their own add-ons, 11046 and 11047.

How many units do I report?

Subtract the first 20 sq cm from the total area actually debrided to subcutaneous depth. Report one unit for each remaining 20 sq cm or part thereof. For 61 sq cm debrided, report 11042 plus three units of 11045.

Can I combine areas from several wounds?

Yes. Add together the areas actually debrided to the same depth. Sum and code areas debrided to different depths separately; do not combine them across depths.

If one wound was debrided to muscle and another to subcutaneous tissue, how is this coded?

For these distinct wounds, code the muscle-depth wound with 11043, plus 11046 if its debrided area exceeds 20 sq cm. Code the subcutaneous-depth wound with 11042, plus 11045 as needed.

What documentation supports additional units?

Record the measured area actually debrided for each wound, using its treated length and width or calculated area. Also record the deepest tissue level actually removed, such as subcutaneous fat. Vague terms like a large wound or extensive debridement do not support units.

When should selective debridement codes be used instead?
