When should 68330 be chosen instead of 68335?

Use 68330 for symblepharon repair without a graft. Use 68335 when the repair includes a free conjunctival or buccal mucous membrane graft.

Can conjunctival adhesion release be billed separately?

Release of the adhesion is part of repairing the symblepharon. Do not separately report 68340 for the same release when it is integral to the repair.

How is bilateral repair reported?

For bilateral surgery, report modifier 50. CMS pays bilateral procedures at 150%.

What documentation supports 68330?

Document the symblepharon, the affected side, and the operative work releasing and repairing the adhesion. The note should make clear that no graft was used.
