0446T billing questions

How is 0446T different from 0447T?

0446T reports insertion and system activation. 0447T is for removal of an implanted glucose sensor without new sensor insertion.

When should 0448T be used instead?

Use 0448T when the service includes removing an existing implanted sensor and inserting a replacement sensor. 0446T is the insertion service without that combined removal.

Can patient training be billed separately?

Training associated with activation of the implanted sensor is part of the 0446T service. Document the instruction provided as part of the insertion and activation.

Can modifier 50 be reported for two implant sites?

No. The CMS bilateral adjustment does not apply, and modifier 50 is inappropriate for this code.

What documentation supports 0446T?

Document the implanted sensor placement, its site, system activation, and patient instruction. The record should distinguish insertion from removal-only or removal-and-replacement services.
