What 69222 covers
An otolaryngologist typically performs this service to clear accumulated material from a mastoidectomy cavity, often called a mastoid bowl, during an office visit. The work may involve removing keratin, crusting, or other debris from the surgically altered cavity. This is distinct from clearing wax or a foreign body from the external ear canal. The code is for complicated cavity debridement; a routine or simple cleaning belongs to the related lower-level service.
Select the code based on the documented complexity of the cavity debridement, not solely on the fact that the patient previously had mastoid surgery. Record the cavity findings, material removed, and work performed to support the service level. It has a 10-day global period, so related postoperative visits during that period are included. When performed with other procedures in the same session, the highest-valued procedure is paid in full and the others are subject to the standard multiple-procedure reduction. For bilateral work, modifier 50 is paid at 150%. Assistant-at-surgery payment is statutorily restricted; co-surgeons and team surgery are not permitted.

Where these rates come from
FeeBase calculates base physician payments from CMS work, practice-expense and malpractice RVUs, adjusted by each locality’s geographic indexes and the conversion factor. Services without a published rate are labeled, never given a made-up amount. Amounts are Medicare allowed amounts before claim adjustments: not a patient’s bill or a commercial rate.
