This service covers supplying a connected medical device that collects and transmits physiologic data, such as blood pressure, weight, pulse oximetry, or blood glucose, from the patient's home. The device provides daily recordings or programmed alert transmissions. It is usually billed by primary care, cardiology, endocrinology, and nephrology practices managing hypertension, heart failure, diabetes, or COPD. Clinical staff or an RPM vendor working under the billing practitioner typically provide the equipment and track whether data arrives. Readings must be captured and sent electronically by the device. Readings the patient types in or reports by phone do not count.

How do I choose between 99454 and 99445?

Count the days with transmitted readings in the 30-day period. Report 99454 for 16 to 30 days and 99445 for 2 to 15 days. Do not report both for the same period.

Can multiple devices generate multiple units of 99454?

No. Report one unit per patient per 30-day period, regardless of how many devices the patient uses. Only one practitioner may bill RPM device supply for that patient during the period.

Do patient-reported readings count toward the 16 days?

No. The device must capture and transmit the data electronically. Values the patient enters manually, phones in, or writes in a log do not qualify.

Can 99454 be billed without 99457?

Yes. Device supply and treatment management are separate services. Report 99457 only when at least 20 minutes of treatment management and the required interactive communication occur during a calendar month.
